Commander Yvonne Gray was navigating the dive support vessel off the south coast of Upolu Island, and ran it aground in Samoa on a known navigational hazard (a reef).
The following day it sank. All 75 crew members were safely evacuated, but the sinking caused an oil spill that released hundreds of thousands of liters of diesel and oil - costing New Zealand $6 million which they had to pay to Samoa in compensation.
Gray was labeled as a DEI hire.
The report indicated the grounding was caused by human error including the crew failing to disengage the autopilot mode.
Additional contributing factors included inadequate training, distracted leadership, and the ship/crew being unprepared for the specific mission.
As of today, three senior officers have been charged under the Armed Forces Discipline Act 1971 and will face court-martial:
Commander Yvonne Gray (the ship's commanding officer) Charged with negligently permitting a ship to be lost (specifically, allegedly being absent from the bridge while within one nautical mile of a known reef), with an alternative charge of negligently failing to perform a duty.
Lieutenant Commander Matthew Gajzago (command supervisor on the bridge):
Charged with negligently causing a ship to be lost (failing to supervise the officer of the watch and prevent the grounding).
A third officer (the officer of the watch):
Charged with negligently causing a ship to be lost, plus negligently permitting a ship to be lost or alternatively negligently failing to perform a duty.

I will have less of a presence on The Torrid Tribe Community. We have very little activity here. This is a sign to me that members have found other social media resources that they are spending more time in. I am happy to see less censorship on social media in general. I started The Torrid Tribe 4 years ago when we were in a state of censorship and lockdowns. It was a difficult time and this was a haven and sanctuary for so many.
I will be lightly posting things here to give you all content to see. This community will always be open to everyone and will resurrect to its full capacity if subscribers show they want it fully operational again with full time administration.
